The rules for issuing a sick leave section filled out by the employer are established by the Procedure for issuing sick leave certificates (approved by order of the Ministry of Health and Social Development of Russia dated June 29, 2011 No. 624n (hereinafter referred to as the Procedure)). The general requirements (clause 65 of the Procedure) are as follows: all entries should be made in type capital letters by hand (black gel, capillary or fountain pen, the use of a ballpoint pen is not allowed) or using printers in specially designated cells starting from the first cell and without going beyond the borders of the cells.

The order of filling out the sick leave line by line

The line "Place of work - name of the organization" indicates the full or abbreviated name of the organization (separate subdivision) in accordance with the constituent documents, or the surname, name, patronymic of the employer - individual(with spaces in one cell). If there is no abbreviated name in the constituent documents or, like the full name, it does not fit into the allotted number of cells, you can arbitrarily shorten the name within the cells of the line (when the information field goes beyond the recording stops). Number signs, quotation marks, dots, commas, dashes and other punctuation marks are not used, but their presence is not a significant design flaw. In any case, the identification of the employer can be carried out by his registration number indicated in the corresponding line of the disability certificate (letters of the FSS of Russia dated October 28, 2011 No. 14-03-18 / 15-12956, dated September 14, 2011 No. -8605, dated 08/05/2011 No. 14-03-11 / 05-8545).

In the lines "Main / Part-time", the mark "V" is placed only in one of these lines, depending on which place of work the certificate of incapacity for work is presented (if the part-time job is internal, a separate sheet with a mark in the column "part-time" is not issued).

The registration number and subordination code are indicated in accordance with the notice issued to the employer by the territorial body of the FSS of Russia on registration as an insured.

The TIN of a disabled person is indicated if available. If the employee does not have a taxpayer identification number, as well as in the disability certificate for pregnancy and childbirth, this line is not filled out. SNILS is required to be indicated (the insurance number of an individual personal account is taken from the mandatory pension insurance certificate issued to the employee).

The line "Calculation conditions" is intended to reflect information about circumstances that can make adjustments to the calculation of benefits, if any, according to the two-digit codes specified in paragraph 66 of the Procedure and on the back of the disability certificate.

The line "Act of form H-1" is to be filled in (day, month and year) when the temporary disability of the employee occurred as a result of an accident at work.

The line "Date of commencement of work" is filled in only in a situation where the employee did not start work on time and his employment contract was canceled, and the disease or injury occurred in the period from the day the employment contract was concluded to the day it was canceled (the date from which the employee had to get to work).

The line "Insurance period" indicates the number of full years and months of the employee's activity, taken into account in the insurance period in accordance with the Rules for calculating and confirming insurance experience to determine the amount of benefits for temporary disability, for pregnancy and childbirth (approved by order of the Ministry of Health and Social Development of Russia dated February 6, 2007 No. 91).

The line "including non-insurance periods" indicates a part of the insurance period in the form of the number of full years and months of the employee's military and other service since January 1, 2007 (Law of the Russian Federation of February 12, 1993 No. 4468-1).

An employee's benefit may be due for the entire period of release from work indicated on the sick leave, or for part of such a period.

The "Period Benefit Due" line indicates the first and last day of paid time. The lines "Average earnings for the calculation of benefits" and "Average daily earnings" indicate the amounts adjusted for the minimum and maximum values. The amount of the daily allowance is not reflected in the disability certificate.

The line "TOTAL accrued" indicates the total amount of the accrued benefit without reducing it by the amount of personal income tax, and the explanations of the Fund with a reverse conclusion (clause 11 of the letter of the FSS of Russia of October 28, 2011 No. 14-03-18 / 15-12956; letter of the FSS of Russia of September 8, 2011 No. 14-03-14 / 15-10022) are applied as based on the previous wording of paragraph 66 of the Order are not subject. In order for the sick leave to be signed not by the head and chief accountant, but by other persons, they must be authorized to do so by a power of attorney issued in accordance with the procedure established by the Civil Code (Article 185 of the Civil Code of the Russian Federation), while indicating the details of the power of attorney, on the basis of which the authorized employee signs a certificate of incapacity for work, not required (clause 12 of the letter of the FSS of Russia dated October 28, 2011 No. 14-03-18 / 15-12956).
